Both entries give the same building address and carry a website address that includes the brand name. One is registered as the second branch of a health centre and the other under the name "Attelia Oral & Dental Health Center", each with its own ST number. The register does not explain how they relate, so ask which one will treat you and quote that number in your contract.

An agency authorisation also does not show that the agency can treat you. If an agency is part of your booking, ask for its legal name and AK number as well.
